Register for your vaccine
- Please make sure to choose an appointment time on the last screen and take a screenshot or print your QR code.
- Bring a photo ID and have the QR code ready when you arrive on-site to verify your registration.
- If you have an insurance card please bring it with you.
- Do not share your QR Code; it can only be used one time by the individual named on the appointment.
You will not be charged for the vaccine.
