General vaccination sites include:

  • Many local pharmacies — such as Kroger, Walgreens, CVS, etc. — provide this immunization.
  • Your general physician’s office can provide you the immunization or information on where to get it.
  • The Dallas County Health and Human Services department offers immunizations for children and adults at discounted rates. Locations and contact information are listed below. For more information, see the health department website.

Where do I submit my meningitis record?

You can submit proof of vaccination through the Med+Proctor portal. It’s easy!

  • Register: Go to medproctor.com and click “register” to create a new account. Enter your email address and follow the directions to get started.
  • Download: Fill out any required personal, medical or insurance information. Download the required forms and follow the directions provided.
  • Upload: Log back in to your Med+Proctor account and upload a copy of your forms. Make sure your forms are complete and legible. You will receive an email confirmation once the forms have been reviewed.

Meningitis is a disease caused by the inflammation of the protective membranes covering the brain and spinal cord known as the meninges. The inflammation is usually caused by an infection of the fluid surrounding the brain and spinal cord. Meningitis is also referred to as spinal meningitis.

If you do not want to receive a meningitis vaccination for reasons of conscience, including religious reasons, you may:

  1. Request an Exemption from Meningococcal Vaccination Requirements for Reasons of Conscience form from the Texas Department of State Health Services
  2. Submit the original, completed, signed and notarized form to Med+Proctor or to your campus Admissions/Registrar’s Office.

Photocopies will not be accepted.

You may decide to complete all your coursework online; however, you will not be able to complete exams or other class projects on campus if your professor requires it. For this option, you must submit a 100% Distance Education Affidavit form.

Important: To enroll for the first time — at some campuses this includes signing up for orientation — you must first complete one of the three options listed above.
